Can you Help to Improve the Intermediate Care Service?
The NHS and Birmingham City Council partners are working together to review and improve care for people who have had a stay in hospital or who are at risk of needing to go into hospital.
There is a particular focus on intermediate care provided in people’s homes (support to help people to recover at home and to regain independence). This service is currently delivered by Early Intervention Community Teams. These are joint teams of Council and NHS professionals.
The service is intended to be short term – lasting no more than six weeks – and focused on achieving personalised outcomes for people who are cared for by the team.
Have you used this service in the last six weeks?
To help improve the service, the NHS and Birmingham City Council partners want to hear from people who have received intermediate care at home. They are interested in speaking to people who have personally received care as well as family and friends who have experience of a loved one who has received this care.
In particular, they want to understand what is important to you in relation to intermediate care. This will help us to improve the service by designing ways of working that are shaped by the experiences of people who have used the service.
People who are current users or have used the service in the last six months and their families/Carers) are invited to join a discussion session to:
• Hear your views about the current service – both the good and the bad elements to share your personal experiences of intermediate care in Birmingham
• To get your views on positive changes we could make
• To work with us ongoing to identify priorities for intermediate care services going forward.
Opportunities to get involved
There are two online sessions for people to join. These will be led by colleagues from the Council and the NHS who work together to deliver intermediate care. These are your opportunities to share first hand your experiences.
These are taking place online on:
• 1st September from 2pm – 3.30pm
• 17th September from 5pm – 6.30pm
