Dementia Action Week. Dementia & Hospitalisation, how Care Coordinators can help.

As part of Dementia Action week, we are running a series of workshops across the week to help raise awareness of the support available.

During this friendly session, you will get the opportunity to find out more about the support that is available to you if your loved one goes into hospital. Meet Pat and Ali who are Care Coordinators at University Hospitals Birmingham – they are a lifeline if the person you care for is admitted to hospital.

The Care Coordinators are there to help with;

• Partner in Care leaflets
